Operation Home Ready

Operation Home Ready (OHR) provides long-term supportive housing for chronically homeless individuals with a documented disability. OHR targets the most vulnerable individuals in the community.

The program employs the best-practice Housing First approach, which aims to increase access to permanent supportive housing and decrease barriers to program entry. Program participants reside in fully furnished one-bedroom apartments throughout Palm Beach County. The program offers weekly case management sessions that initially focus on managing tenancy. Case managers meet clients in their homes or in the community.

As clients continue to participate in the program, additional services and interventions are offered, including money management, life skills, establishing community relationships, self-care, and participation in meaningful daily activities.

An interdisciplinary team that consists of medical and mental health professionals, SSI/SSDI Outreach, Access, and Recovery (SOAR) program staff, an Organizational Representative Payee, and Peer Support Specialists that provide services that assist and support participants as they recover from their long history of homelessness.

The program is currently equipped to serve up to 57 formerly chronically homeless Palm Beach County Residents.
