Operation Home Ready

Operation Home Ready (OHR)/Home First provides long-term supportive housing for chronically homeless individuals with a documented disability and a willingness to be housed. OHR targets the most vulnerable individuals in the community.

The program employs the best-practice Housing First approach, which aims to increase access to permanent supportive housing and decrease barriers to program entry. Program participants reside in fully furnished one-bedroom apartments throughout Palm Beach County. The program offers wraparound case management services that initially focus on managing tenancy. Case managers meet clients in their homes or in the community on a weekly or bi-weekly basis, depending on the need.

As clients continue to participate in the program, additional services and interventions are offered, including money management, life skills, establishing community relationships, self-care, and participation in meaningful daily activities.

An interdisciplinary team that consists of medical and mental health professionals, as well as Recovery (SOAR), and Peer Support Specialists who provide services that assist and support participants as they recover from their long history of homelessness.

Coordinated entry refers all participants to the housing program.  Community access to housing is unavailable for the OHR/Home First programs.  In addition, the program is currently equipped to serve up to 85 formerly chronically homeless Palm Beach County residents.
