We are seeking an exceptional, self-motivated and compassionate individual to join our Women’s Services team to provide comprehensive case management services for homeless individuals recently coming out of incarceration in a residential housing program. The program includes a social enterprise component and employment goals will be attached. Case management functions include, but are not limited to the following: individual assessment and problem identification, linking residents to needed services, service implementation and coordination with timely and thorough documentation, daily monitoring, resident advocacy, evaluation and follow-up. Hours may vary including evenings and weekends.
Some essential functions and responsibilities include:
- Be highly visible and engage with residents to assure a safe and sober community.
- Provide crisis intervention, monitoring and risk management to high-risk clients.
- Provide case management, resource linkage, support and advocacy for program participant.
- Conduct groups and life skill workshops.
- Assist residents in developing service plans.
- Secure all necessary documentation to verify income from program, entitlement programs and other sources.
- Develop, in partnership with residents, a realistic savings schedule and budgeting goals. Reviewing basic budgeting skills for assistance toward self-sufficiency.
- Collect savings, document and submit cash collected to Director of Women’s Services for deposit.
- Maintain timely documentation and case notes in resident file.
- Conduct initial evaluations for residents seeking mental health services.
- Follow-up with referrals made and advocate on behalf of residents when appropriate.
- Keep accurate records of residents regarding attendance of required meetings.
- Keep current on resources available within the community by attending workshops, conferences and meetings as assigned by the Director of Women’s Services.
- Assist residents with employment goals and resume preparation.
- Become certified for case management within the first 90 days of employment.
- Utilize and update resident information on the agency database.
- Create with residents’ participation, a monthly calendar of Halle Place social events.
- Perform apartment inspections on a daily basis and random drug testing. Report apartments that do not meet minimal expectations to the Director. Report U/S results to the Director.
- Contact Director for guidance with resident issues as needed.
- Maintain current CPR/First Aid certification (paid for by The Lord’s Place).
- Accept donations when available, providing written receipts to donors using a standard donor receipt booklet.
- Perform additional duties as assigned by the Director of Women’s Services according to business needs.
Requirements for the position include:
- Bachelor’s degree in social work, psychology, mental health counseling or related field.
- Previous shelter administration experience strongly preferred.
- Excellent verbal and written communication skills required.
- Computer skill, including a working knowledge of Office 97 or higher required.
- Requires ability to operate business equipment used daily within the organization.
- Requires ability to learn and use whatever documentation system appropriate to their program, including electronic databases to document case notes.
- Requires completion of certification training within the first 90 days of employment.
- May require safely lifting up to 15 lbs.
- Maintain a valid Florida State drivers’ license with a satisfactory driving record and eligibility for coverage under any applicable Company insurance.
- Requires some walking to inspect grounds and facilities (approximately 500 yards per day which may be done at staggered times).
- Donations are frequently received. Ability to lift or pull 50 lbs. if necessary (handcart may be used).
