We are looking for a Recovery Coordinator to join our Clinical Services team. This position coordinates and provides substance use recovery support to adults, and works alongside the Director of Clinical Services and housing programs to coordinate entry into The Lords Place programs as needed. This position will work with the agency’s peer specialists and clinical staff to ensure quality and consistency in the provision of recovery related services and support. The Recovery Coordinator will provide direct services and support to participants in need of additional support with substance use recovery, relapse prevention and/or harm reduction as appropriate based on individual needs. They will also maintain current knowledge of innovative practices in supporting individuals in recovery and will participate in and facilitate trainings. Hours may vary including evenings and weekends.
Some essential functions and responsibilities include:
- Provide substance use and secondary mental health assessments, treatment planning and/or linkage to counseling services to primarily chemically addicted populations.
- Participate in clinical team meetings, training sessions and committee meetings as assigned.
- Plan, structure, and organize different types of group therapy sessions.
- Develop service and continuing care plans.
- Participate as a member of the Mental Health Team and coordinates with other facility staff.
- Experience working with a corrections population is desired along with substance abuse treatment experience.
- Complete and maintain accurate records or reports regarding the clients’ histories and progress, services provided, or other required information.
- Initiate referrals for needed services that are not directly provided by the program.
- Serve as on-site Substance Abuse liaison of the organization.
- Develop and implementation of individualized treatment plans in conjunction with patients and multidisciplinary treatment team.
- Coordination of services with continuing care providers.
- Document and complete assessments, service plans, progress reports, case correspondence, and other required monitoring and reporting documentation as appropriate and on a timely basis.
- Facilitate or co-lead small groups of consumers/or significant others to increase understanding of the consumers issues or to improve coping skills and performance of activities of daily living.
- Maintain regular contact with consumers during inpatient hospitalizations.
- Provide substance use case management services to a caseload of seriously and persistently mentally ill consumer/patients.
- Through regular contact with consumers and providers, monitor consumer’s participation and satisfaction in all services used by consumers on their caseload, and determine if the individual is progressing and if specific services continue to be needed and are appropriate.
- Experience of working with clients suffering with substance use issues and writing reports or evaluations.
Requirements for the position include:
- High School Diploma or GED required.
- Bachelor’s Degree in Psychology, Social Work, Certified Addiction Professional, or Counseling preferred and/or extensive experience with addiction, linkage to support services and follow through.
- Must be able to pass Level II DCF clearance.
- Familiarity with local support groups and agencies that provide services preferred.
- Excellent verbal and communication skills.
- Computer skills, including a working knowledge of Office 97 or higher required.
- Ability to operate business equipment used daily within the organization.
- Ability to learn and use whatever documentation system appropriate to their program, including electronic databases to document case notes.
- Completion of SOAR, SPDAT and certification training within the first 90 days of employment.
- May require safely lifting up to 15 lbs.
- Maintain a valid Florida State drivers’ license with a satisfactory driving record and eligibility for coverage under any applicable Company insurance.
